Incident

An incident is an event that threatens the confidentiality, integrity, or availability of systems, data, or services and requires a coordinated response.

An incident is an event or set of related events that creates a meaningful security, operational, customer, or compliance concern and needs a managed response.

An incident record should make clear:

  • What was observed and when
  • Which asset, account, service, or data was affected
  • What impact is known or suspected
  • Who owns the response
  • Which actions and decisions have been made
  • What evidence supports the current assessment

Not every alert becomes an incident. Teams normally validate the signal, assess impact, and decide whether coordinated response work is needed. Keeping that decision visible helps prevent both overreaction and missed escalation.
